Cardiac metabolismAlthough the heart is one of the main energy consuming organs of the body and despite the profound changes in its metabolism in the failing heart, cardiac metabolism has been seldom considered as a potential treatment target.A major change in the cardiac metabolism occurs in the failing heart with the main source of ATP production shifting from fatty acid oxidation to the more efficient glucose oxidation and, then, to the far less efficient anaerobic glycolysis.Changes in myocardial metabolism may contribute to the impairment in contractility and heart failure (HF) progression.Many drugs may have potential beneficial effects on myocardial energy production and improve cardiac contractility.As outlined in the review by Heggermont et al. they may act through blockade of fatty acid oxidation, stimulation of glucose oxidation or through indirect approaches such as the increase in calcium reuptake or stronger actin myosin cross-bridge formation or better mitochondrial energy production. 1
