Analyse der Bronchiallavage bei interstitiellen Lungenerkrankungen

Abstract
Unter dem Begriff interstitielle Lungenerkrankungen (ILD) werden zahlreiche Krankheiten zusammengefasst, die im fortgeschrittenen Stadium zum Bild der Lungenfibrose führen können. Am häufigsten (50%) ist die Sarkoidose, gefolgt von der exogen allergischen Alveolitis (EAA), den Pneumokoniosen und der UIP (usual interstitial pneumonia) / IPF (idiopathic pulmonary fibrosis) mit jeweils 10%. Therapeutische Ansätze zur Behandlung interstitieller Lungenerkrankungen bestehen im Einsatz von Kortikoiden und Immunsuppressiva, die Prognose ist insgesamt eher schlecht. Zur Entwicklung besserer Behandlungsmöglichkeiten ist das Verständnis der pathophysiologischen Vorgänge in der Lungenmatrix wichtig. Hierbei spielen bestimmte Zytokine, wie z.B. TGF-ß1 eine große Rolle. Hypothese: TGFß1 unterstützt durch Aktivierung von Proteaseinhibitoren (TIMP=tissue inhibitors of metalloproteinases) und Hemmung von Metallopreoteinasen (MMP=matrix metalloproteinases) eine Störung der Gewebe-Remodelings. Folge ist die Entwicklung einer Lungenfibrose. Durch Messung dieser Parameter in der Bronchiallavage von Patienten mit interstitiellen Lungenerkrankungen kann eine Aussage über die Dynamik der Fibrosierung gemacht werden. Ergebnisse: Wir untersuchten bei verschiedenen Patientenkollektiven (UIP, Kollagenosen, Vaskulitiden, COPD, Pneumonien, Sarkoidose und übrige ILD) die Flüssigkeit der BAL und quantifizierten u.a. die Menge von TGFß1, MMP-8, MMP-9 und TIMP-1. Signifikant höhere Werte fanden sich bei Patienten mit UIP bzw. ILD unbekannter Ätiologie im Vergleich zu Sarkoidose und der Kontrollgruppe. Kein Unterschied fand sich im Vergleich der UIP mit Pneumonien, COPD oder der EAA. Interessant wird im weiteren die Korrelation mit Lungenfunktionsparametern dieser Patienten sein.
