Abstract
زمينه و هدف: بالون پمپ داخل آئورت(Intra Aortic Ballon Pump=IABP) يک دستگاه مکانيکی است که جهت کمک به حفظ جريان خون سيستميک مورد استفاده قرار میگيرد و يک درمان پذيرفته شده در بيمارانی که علايم هموديناميک آنها چه قبل و چه بعد از عمل جراحی شديدا تحت مخاطره قرار گرفته، محسوب میشود. اين مطالعه جهت بررسی عوامل تعيين کننده زنده ماندن يا زنده نماندن بيمارانی که بعد از جراحی قلب به بالون پمپ نياز دارد انجام شده است. روش بررسی: اين مطالعه به صورت گذشتهنگر بر روی 46 بيمار از 2200 بيماری که طی يک سال تحت عمل جراحی بای پس عروق کرونر(Coronary Artery Bypass Grafting=CABG) در بيمارستان شهيد رجايی تهران قرار گرفتهاند و نياز به بالون پمپ پيدا کردهاند انجام گرفته است. اين بيماران به دو گروه: شامل گروهی که زنده ماندند(33 نفر) 2- گروهی که فوت کردند(13 نفر) تقسيم شدند، و گروه دوم به دو زير گروه بيمارانی که از بالون پمپ جدا شدند و بيمارانی که از بالون پمپ جدا نشدند، تقسيم گرديدند. متغيرهای بررسی شده برای هر بيمار عبارتند از: سن، جنس، سطح بدن، سيگاری بودن، سابقه انفارکتوس ميوکارد(Myocardial Infarction=MI) ديابت قندی، هيپرتانسيون، ميزان کسر خروجی خون از بطن چپ (Ejection Fraction=EF) ميزان فعاليت افراد بر حسب معيارهای انجمن قلب و عروق کانادا(Canadian Cardiovascular Society=CCS) سابقه عمل جراحی قلب، مدت زمان گردش خون برون پيکری(Cardio Pulmonary Bypass=CPB)، کلامپ آئورت(Aortic Cross Clamping=ACC)، وابستگی به بالون پمپ، تهويه مکانيکی و طول مدت بستری در بخش مراقبتهای ويژه(Intensive Care Unit=ICU). اختلاف بين دو گروه به وسيله chi-Square و Student test آناليز شده 05/0p ). از بيمارانی که زنده نماندند، 6 بيمار از بالون پمپ جدا شده و 7 بيمار از آن جدا نشدند ولی در نهايت همه آنها به دليل نارسايی قلبی، 009/0=p نارسايی کليوی 059/0=p و نارسايی تنفسی 3/0=p فوت نمودند. نتيجهگيری: از اين مطالعه میتوان نتيجه گرفت که مهمترين عامل مرگ در بيمارانی که از بالون پمپ جدا نشدند نارسايی قلبی بوده و بيمارانی که ميزان کسر خروجی خون از بطن چپ کمتر از 30 درصد(30%EF<) داشتهاند پيشآگهی بدتری را دارا بودند. در بيمارانی که از بالون پمپ جدا شدند به ترتيب نارسايی کليه، نارسايی قلبی و تنفسی عامل تعيين کننده مرگومير آنها بوده است.
