Abstract
목적: 뇌척수액 누출로 경막외 혈액 색전술을 받은 환자들을 대상으로 뇌척수액 누출의 원인이 되는 진단명과 누출부위(leakage of spine level)에 따른 색전술의 치료 성과(Clinical success)를 평가하였다. 대상 및 방법: 2015년 1월부터 12월까지 본원에서 뇌척수액 누출로 인한 경막외 혈액 색전술을 시행한 56명의 환자를 대상으로 통증을 평가(NRS)하고 진단명(경막천자 후 두통, 자발성 두개내 저압, 경막하 혈종)과 Spine 누출 부위를 구분하여 후향적(Retrospective) 연구를 하였다.1ST 색전술 후 증상의 완전 경감(complete relief)이 있을 시 치료성과(Clinical success)가 있는 것으로 평가하였으며 실험의 통계적 유의성 평가를 위해 IBM SPSS statistics v18.0(Ontario, Canada)의 신뢰도분석(Reliability analysis)과 one-way ANOVA, Multiple regression analysis을 사용하여 치료성과에 영향을 주는 인자를 분석하였다. 결과: 환자들의 NRS Score는 평균 7점으로 Severe pain에 해당하였고 진단 별 치료 성과는 경막천자 후 두통 환자가 89%로 가장 높았으며 각 진단 별 비교 분석으로 경막하 혈종을 제외한 나머지 두 진단 군에서는 유의성이 있는 것으로 판단되었다. (p=0.006) 누출 부위에 따른 치료 성과는L-spine에서 82%으로 가장 높았으며 여러 부위에서 누출이 있는(Multi-level spine) 환자의 경우 1ST 색전술로 증상 경감 효과가 낮은 것으로 분석 되었으며 누출 부위 별 비교분석 결과 C-spine 제외한 나머지 두 누출 부위군에서는 유의성이 있는 것으로 판단되었다. (p=0.001) 치료성과 (Clinical success)의 영향인자(성별과 나이, 색전술 시행횟수, 진단명, 누출범위) 5가지를 Multiple regression analysis 분석 결과 시술 횟수가 많아질수록 치료 성과는 높아진다고 판단되었다. (p=0.001) 결론: 자가 혈액을 이용한 색전술의 치료 성과(Clinical success)는 요추 천자(spine level L3/4)로 발생한 경막천자 후 두통 환자에게서 높은 성공률을 보였으며 뇌척수액 누출환자의 진단과 누출부위(leakage of spine level)를 고려하여 실시한다면 효과적인 치료법이다.
