Impact of IV insulin in diabetic patients on FDG-PET studies

Abstract
50 Objectives: To evaluate the effectiveness of a standardized insulin protocol in reducing plasma glucose level (PGL), review FDG biodistribution using such a protocol and assess the clinical impact. Methods: FDG-PET studies from 4593 clinical cancer patients were reviewed from July 2003 to October 2005. Among these, 63 patients with PGL>10 mmol/L, evaluated for lung cancer in 52% of cases, received IV insulin (Humulin R) doses according to a standardized protocol. 106 consecutive euglycemic patients ( Results: PGL decreased from 13±2 to 7±2 mmol/L after insulin injection. Images from patients who received insulin showed significantly more MU compared to control group (scores 1.6±1.5 vs. 0.4±0.6 p Conclusions: A standardized protocol using IV insulin prior to FDG injection in diabetic cancer patients can be safe and effective to reduce PGL. However, physicians should be aware of potentially inadequate biodistribution in as much as 25% of patients, even thought this did not appear to affect the overall diagnostic sensitivity. In addition to visually increased MU, a low hepatic FDG uptake was a good indicator of inadequate biodistribution.
