Abstract
Bol neuropatyczny stanowi duze wyzwanie diagnostyczne i terapeutyczne. Pomimo znacznego postepu w zakresie poznania jego mechanizmow i wielu badan oceniających skutecznośc i bezpieczenstwo stosowania lekow w bolu neuropatycznym jego leczenie jest nadal trudne. W praktyce rozpoznawany jest rzadziej niz w badaniach epidemiologicznych, a u wielu chorych nie udaje sie osiągnąc satysfakcjonującego wyniku leczenia. Rozpoznanie bolu neuropatycznego moze byc ustalone na podstawie wywiadu i badania przedmiotowego, ze szczegolnym uwzglednieniem zaburzen czucia. Do najczestszych i najlepiej poznanych zespolow obwodowego bolu neuropatycznego zalicza sie bolesną neuropatie cukrzycową, neuralgie trojdzielną i popolpaścową. Lekami pierwszego wyboru w farmakologicznym leczeniu bolu neuropatycznego są: trojpierścieniowe leki przeciwdepresyjne, inhibitory zwrotnego wychwytu serotoniny i noradrenaliny, gabapentyna, pregabalina, opioidy i lidokaina w plastrach. W ostatnich latach ukazaly sie szczegolowe rekomendacje dotyczące leczenia bolesnej polineuropatii cukrzycowej opracowane przez Amerykanskie Towarzystwo Diabetologiczne, Europejską Federacje Towarzystw Neurologicznej, Amerykanską Akademie Neurologii i wielospecjalistyczną grupe ekspertow europejskich i amerykanskich ( the Toronto Consensus Panel on Diabetic Neuropathy ). Zalecają one w tej neuropatii stosowanie TLPD, a2-d ligandow kanalu wapniowego i SNRI, jako lekow pierwszego wyboru. Autorzy wytycznych zgodnie podkreślają, ze leczenie neuropatii cukrzycowej jest trudne, a zadowalający efekt analgetyczny udaje sie uzyskac zaledwie u okolo 50% chorych.
