Identification d'auto-antigènes et caractérisation d'auto-anticorps dans les hépatites auto-immunes

Abstract
SOMMAIRELes auto-anticorps anti-microsomes de foie et de rein de type 1 (anti-LKM1) sont les marqueurs sérologiques de l'hépatite auto-immune (HAI) de type 2. Ces anticorps sont dirigés contre le cytochrome P45011D6 (CYP2D6).Deux types d'auto-anticorps ont été identifiés au cours de l'HAI de type 2; les premiers sont dirigés contre les épitopes linéaires et les seconds sont spécifiques aux épitopes conformationnels de la protéine CYP2D6.Ceci nous indique que les anticorps anti-LKM1 sont le résultat d'une réponse polyclonale, cette réponse induite et maintenue par l'auto-antigène CYP2D6, probablement suite à une lyse des hépatocytes.Les anticorps anti-LKM1 sont associés dans 30% des cas aux anticorps anti-cytosol (anti-LC1); Ces derniers peuvent cependant être les seuls marqueurs sérologiques chez 10% des patients atteints d'HAI de type 2. Nous avons identifié l'auto-antigène reconnu par les anticorps anti-LC1 par immuno-criblage d'une banque d'ADNc des cellules HepG2.Le séquençage du clone LCHC1 (liver cytosol human clone 1) nous a permis d'identifier l'auto-antigène comme étant une enzyme hépatique, appelée la formiminotransférase cyclodésaminase (FTCD).Un épitope principal reconnu par tous les sérums de patients anti-LC1 positifs a été identifié.Cet épitope formé de 146 acides aminés est localisé dans la région C-terminale de la protéine.L'identification de la protéine FTCD comme l'auto-antigène reconnu par les anticorps anti-LC1 est une étape importante dans le développement d'un test de diagnostic additionnel pour le dépistage de l'HAI de type 2. Ce test est d'autant plus spécifique lorsque les anticorps anti-LC1 sont les seuls marqueurs sérologiques de la maladie.S'il est bien admis que les anticorps anti LKM1 et anti-LC1 sont des marqueurs sérologiques qui permettent de caractériser un sousgroupe de patients atteints d'HAI; les auto-antigènes CYP2D6 et FTCD reconnus respectivement par les anti-LKM1 et anti-LC1 sont cependant des protéines intra-cellulaires non-exposées aux auto-anticorps circulants.La responsabilité de ces auto-anticorps dans la pathogenèse de l'HAI de type 2 reste encore à établir.En revanche, des auto-anticorps dirigés contre des protéines antigéniques de la membrane des hépatocytes peuvent, par contre, expliquer la lyse cellulaire souvent observée chez les patients atteints d'HAI.Dans ce but, nous avons isolé et purifié le récepteur des asialoglycoprotéines (ASGP-R) à partir de foie humain et nous avons mis au point un test ELISA.Ce test nous a permis de détecter la présence sérologique d'auto-anticorps anti-ASGP-R chez 75% d'enfants atteints d'HAI de type 1 et 40% d'enfants atteints d'HAI de type 2. Nous avons montré que les anticorps anti-ASGP-R sont dirigés contre des épitopes conformationnels de la structure hétéro-oligomérique de la protéine mature isolée à partir de foie humain.Les chaînes de carbohydrates de la protéine sont situées dans le domaine extra-cellulaire de la membrane des hépatocytes, elles sont ainsi exposées et accessibles aux auto-anticorps circulants.Ces chaînes font partie des épitopes conformationnels.Nous pouvons conclure suite à ces résultats que les auto-anticorps anti ASGP-R pourraient jouer un rôle dans la pathogenèse de l'HAI.Certains xénobiotiques peuvent induire un tableau clinique et biologique semblable à une HAI.Dans cette thèse, nous rapportons le cas d'une adolescence atteinte d'une hépatite induite par la minocycline.Nous avons montré dans le sérum de cette patiente la présence d'anticorps dirigés contre différentes protéines des hépatocytes en particulier les cytochromes P450.L'hépatite observée chez cette patiente pourrait être la conséquence d'une réaction immune induite par un traitement à la minocycline.Bien qu'il est possible d'envisager un mécanisme semblable à celui observé dans les cas des hépatites induites par l'acide tiénilique, la dihydralazine ou l'halothane.Le métabolite réactif responsable de cette hépatotoxicité n'a pas encore été identifié.Mots clés : Hépatites auto-immunes.Anti-microsomes de foie et de rein de type 1. Su mm a ryAnU-Iiver-kidney microsome type-1 antibodies (LKM1), present in sera from patients with AIH type 2 react specifically with CYP2D6.The autoimmune response against CYP2D6 is directed against linear and conformational antigenic sites.These resuits strengthen the argument that the LKM-1 response is polyclonal and antigen driven.These findings could be interpreted to mean that the unmodified antigen must be present within hepatocytes and is liberated after liver injury.Anti-liver cytosol type 1 autoantibodies (LC1) have been reported in association with LKM1 autoantibodies in 30% of patients with AIH type 2. In 10% of cases, LC1 antibodies are the only Iiver-related circulating autoantibodies.After the screening of cDNA Iibrary with anti-LC1-positive sera, we identified human liver formiminotransferase cyclodeaminase (FTCD) as the specific antigen recognized by LC1 antibodies.The liver cytosol human clone 1 (LCHC1) codes for a peptide of approximately 150 amino acids from the FTCD C-terminal region.AIl the anti-LC1-positive sera, as detected by indirect immunofluorescence and/or immunodiffusion, reacted against the LCHC1 ptotein by immunoblot, suggesting that this portion of the antigen contains a major linear epitope.Dur current work is directed towards the development of more specific and faster diagnostic tests than those so far used for the detection of anti-LC1s.The role of LKM1 or LC1 autoantibodies in the pathogenesis of the disease is controversial, as both are directed against intracellular antigens.Liver-specific antigens exposed on the hepatocyte cell surface may play a role in the pathogenesis of AIH.The asialoglycoprotein receptor (ASGP-R) is an integral membrane protein of the sinusoidal domain of the hepatocyte plasma membrane and anti-ASGP-R autoantibodies might play a role on the pathogenesis of AIH.We purified human ASGP-R from liver and used as antigen in ELISA test to detect anti-ASGP-R antibodies in the sera of patients with autoimmune hepatitis.Anti-ASGP-R antibodies were detected in 75% of chiidren with AIH type 1 and 40% with AIH type 2. Dur results show that ASGP-R antibodies in sera of patients with AIH are directed against conformational epitopes of the mature hetero-oligomeric form of the human receptor and that at least some of the epitopes are located on the extracellular domain of the antigen and are readily accessible to circulating autoantibodies.These finding lead to the speculation that anti-ASGP-R autoantibodies might play a role in the pathogenesis of AIH.In this report we describe a case of chronic active hepatitis secondary to minocycline treatment for acne in a female adolescent.The patients serum immunoprecipited two proteins of 50 kDa and 90 kDa respectively, from cysteine S35 labeled human hepatoma cells.Furthermore the patients serum recognizes a 5OkDa protein of rat liver microsomes when tested by western blot, and different purified P450 proteins by ELISA test.The mechanism by which an autoimmune reaction develops is not known.We speculate that there must be an antibody reaction against an as yet unknown metabolite of minocycline which cross-reacts with several microsomal cytochromes.Furthermore, this finding suggests that the mechanism of toxicity could be analogous to that described in immunoallergic reaction secondary to ingestion of halothane, tielinic acid or dihydralazine.Keys words: Autoimmune hepatitis.Liver-kidney microsome antibody type 1.
