Abstract
A previously healthy 13-year-old girl initially presented to the emergency department (ED) with a 2-day history of fever, headache and eye pain. Aside from a temperature of 38.6°C, her physical examination was unremarkable. Her complete blood count was normal, C-reactive protein (CRP) was 13.3 mg/L and monospot test was negative. She was discharged home with a presumed diagnosis of viral illness. She presented again to the ED on day 4 of illness with persistent fevers, left hip and buttock pain, limp and emesis. Her vital signs were normal. She had pain with range of motion of her left hip. Her peripheral white blood cell count was 14.9 × 109/L with predominance of bands 2.4 × 109/L. CRP had increased to 43.5 mg/L. X-ray of the left hip and ultrasound did not show a fracture or effusion. She was discharged home with presumed viral synovitis. Past medical history and family history were noncontributory.
