Abstract

Fragestellung: Die postprandiale Blutzuckereinstellung (PPG) bei Menschen mit Diabetes ist immer noch unzureichend. Schnell wirksames Insulin aspart (Faster aspart) ist Insulin aspart (IAsp) in einer neuen Formulierung, mit dem Ziel einer rascheren Insulinresorption. Diese multizentrische Treat-to-Target-Studie untersuchte Wirksamkeit und Sicherheit von Faster aspart bei Erwachsenen mit Typ 1 Diabetes. Methodik: Nach einer 8-wöchigen Run-in-Phase zur Optimierung der Basalinsulinbehandlung, wurden die Patienten (mittlerer HbA 1c 7,6%) doppelblind auf Faster aspart (n = 381) oder Insulin aspart (IAsp; n = 380) zu Mahlzeiten oder offen auf Faster aspart nach Mahlzeiten (20 min nach Beginn der Mahlzeit; n = 382) randomisiert, jeweils in Kombination mit Insulin detemir. Der primäre Endpunkt war die Veränderung des HbA 1c gegenüber Baseline nach 26 Wochen. Ergebnisse: Der HbA 1c wurde in allen drei Armen gesenkt mit Nichtunterlegenheit gegenüber IAsp bei der Gabe zur und nach Mahlzeitenbeginn (geschätzter Behandlungsunterschied [ETD], [95% KI]: zur Mahlzeit, -0,15 [-0,23; -0,07] %); nach Mahlzeitbeginn, 0,04 [-0,04; 0,12]); die HbA 1c -Senkung war für Faster aspart zur Mahlzeit signifikant größer als für IAsp. Faster aspart zur Mahlzeit war IAsp hinsichtlich des 2-h-PPG-Anstiegs im standardisierten Mahlzeitentest überlegen (ETD: -0,67 [-1,29; -0,04] mmol/l). Der 1-h-PPG-Anstieg war ebenfalls geringer (ETD: -1,18 [-1,65; -0,71] mmol/l). Die Raten schwerer oder mittels Blutzuckermessung bestätigter Hypoglykämien (Plasmaglucose < 3,1 mmol/l) waren vergleichbar, bei einem insgesamt vergleichbaren Sicherheitsprofil. Schlussfolgerung: Faster aspart zur Mahlzeit bewirkte gegenüber IAsp eine signifikant stärkere Reduktion des HbA 1c und eine überlegene PPG-Einstellung. Die Blutzuckereinstellung für postprandial verabreichtes Faster aspart war mit IAsp zur Mahlzeit vergleichbar. Dies stellt einen klinischen Fortschritt bei der Behandlung von Typ 1 Diabetes dar.
