Abstract
A previously healthy 7-year-old girl presented to the emergency department with acute onset esotropia of the left eye associated with left eye pain, which had been present since that morning. There was no history of night sweats, fever or weight loss, nor a history of headache, vomiting or focal neurological deficits. She did not have any sick contacts or recent travel. On physical examination, she was afebrile, and appeared well. Her left eye was turned inwards (esotropia) with a reduced ability to gaze laterally with the left eye. The extraocular movements of her right eye were intact. There was no papilledema on examination, and the remainder of her cranial nerves were intact. Her neurological examination was otherwise completely normal. On examination of her head and neck, it was noticed that her left tympanic membrane was erythematous and perforated, with some purulent drainage. In addition, erythema was noted over the skin behind her left ear. The remainder of her physical examination was unremarkable.
