Regression der linksventrikulären Muskelmasse durch Gewichtsabnahme ohne und mit Sibutramin – eine Multicenter-Studie

Abstract
Zielsetzung: Eine linksventrikuläre Hypertrophie (LVH) ist bei Adipositas häufig, besonders bei Präsenz einer Hypertonie. Durch Gewichtsabnahme kommt es zu einer Regression der linksventrikulären Muskelmasse (LVM). In welchem Maße dies unter Sibutramin bei Patienten mit einer LVH der Fall ist, ist nicht bekannt. Material und Methoden: 195 Adipöse wurden 12 Wochen entweder mit 15mg/d Sibutramin oder Placebo behandelt. Die Patienten waren 18–65 Jahre alt, hatten einen BMI von 30–40kg/m 2 und eine LVM von >190g (Männer) bzw. >160g (Frauen). Die Körperzusammensetzung wurde durch BIA und der Blutdruck ambulant über 24 Stunden ermittelt. Die kardiologischen Paramater wurden mittels einer Echokardiographie im M- und D-Mode erhoben. Ergebnisse: Die Gewichtsabnahme betrug 2,1kg unter Placebo und 6,9kg unter Sibutramin, die Körperfettabnahme lag bei 1,6kg bzw. 5,2kg. Der Blutdruck nahm unter Placebo um 6,9/3,3mm Hg ab, unter Sibutramin gab es keine signifikanten Änderungen. Die Herzfrequenz änderte sich in beiden Gruppen unterschiedlich; unter Placebo sank sie um 1,6 Schläge/Min. und unter Sibutramin stieg sie um 5,3 Schläge/Min. an. Die LVM und der LVM-Index nahmen unter Placebo nicht ab, unter Sibutramin wurden sie um 10,60g bzw. 2,14g/m 2 reduziert. Änderungen der LVM waren mit der Gewichtsabnahme korreliert, nicht jedoch mit Änderungen des Blutdrucks oder der Herzfrequenz. Die systolische und diastolische Herzfunktion änderten sich marginal und ohne Unterschied zwischen beiden Gruppen. Zusammenfassung: Sibutramin kann nicht nur zur Gewichtsreduktion, sondern auch zur Regression der LVM bei adipösen Patienten mit und ohne Hypertonie empfohlen werden.
