Abstract
胸膜肺全摘を施行せず，2年間無再発生存中の限局性悪性胸膜中皮腫の1例を経験したので報告する．本疾患は，病理学的にはびまん性悪性胸膜中皮腫と全く同じ所見を持つものの，生物学的悪性度が異なる稀な疾患である．症例は元建設業の70代男性．他疾患で受診した際，胸部レントゲンで左上肺野に約10 cm大の腫瘤を指摘．経皮的穿刺細胞診でclass V腺癌と診断された．胸壁合併左肺上葉切除を施行．病理学的に二相型悪性胸膜中皮腫と診断された．追加の治療は施行せず術後2年余経過したが，CTやPETで再発所見なく現在も生存中である．本疾患は術前から「限局性」と診断することが困難であり，術式選択が最大の問題となる．症例を集積し，診断方法を確立する必要がある．
