Aneurysmatische SAB: Höheres Komplikationsrisiko nach Clipping oder Coiling?

Abstract
Eine aneurysmatische Subarachnoidalblutung (SAB) ist ein lebensbedrohendes Ereignis. Vorangegangene Studien zeigten, dass Patienten mit geclippten Aneurysmen häufiger neurokognitive Defizite und Anfälle erleiden als Patienten, die sich einem Coiling unterziehen. Ob die Inzidenz von anderen Komplikationen während des Krankenhausaufenthalts beim Clipping ebenfalls höher ist, haben M. D. I. Vergouwen et al. in einer Studie untersucht. Die Untersuchung basierte auf Daten des „Registry of the Canadian Stroke Network“. Infrage kamen alle Patienten, die zwischen Juli 2003 und März 2008 aufgrund einer aneurysmatischen SAB in ein Krankenhaus aufgenommen wurden und sich dort einem Clipping oder Coiling unterzogen. Patienten, die innerhalb von 2 Tagen nach Aufnahme starben, gingen nicht in die Studie ein. Die Autoren verglichen Patienten mit geclippten und gecoilten Aneurysmen in Bezug auf bestimmte Charakteristika zu Studienbeginn, die Inzidenz von Komplikationen im Krankenhaus (bis 30 Tage nach Aufnahme), die Aufenthaltsdauer, ein schlechtes funktionelles Ergebnis (Score auf der „Modified Rankin Scale“ bei Entlassung von ≥ 3) sowie Mortalität. Insgesamt nahmen 931 Patienten an der Studie teil. Das Durchschnittsalter betrug 55,4 Jahre, 68,2 % waren Frauen. Bei 548 Probanden (59 %) wurde ein Clipping durchgeführt, und 383 (41 %) unterzogen sich einem Coiling. Gegenüber Patienten mit gecoilten Aneurysmen wiesen solche mit einem Clipping eine höhere Inzidenz von Komplikationen während des Krankenhausaufenthalts (37,2 vs. 24,5 %; p < 0,0001), ein schlechtes funktionelles Ergebnis bei Entlassung (69,4 vs. 51,4 %; p < 0, 0001), eine höhere Sterblichkeit (14,6 vs. 9,1 %; p = 0,01) sowie eine längere Verweildauer auf (17 vs. 13 Tage; p < 0,0001). Innerhalb der „Clipping-Gruppe“ waren die Inzidenzen von Harnweginfektionen (p = 0,02), Pneumonie (p = 0,01), Herz- und Atemstillstand (p = 0,007), Anfällen (p = 0,01) und Druckgeschwüren (p = 0,02) höher als innerhalb der „Coiling-Gruppe“. Dies traf aber beispielsweise nicht auf tiefe Venenthrombose, Sepsis, Myokardinfarkt und Lungenembolie zu. Bei Harnweginfektionen, Pneumonie, Herz- und Atemstillstand sowie Anfällen handelte es sich um unabhängige Prädiktoren für ein schlechtes funktionelles Ergebnis.
