Cerebral Oxygenation During Defibrillation Threshold Testing of ICDS

Abstract
To evaluate cerebral cortical oxygenation and cellular homeostasis during defibrillation threshold (DFT) testing of ICDs by monitoring specific chromophores which reflect intravascular and mitochondrial levels of oxygenation. 11 patients (male = 9: age 64±11 yrs: LVEF 44±11%) underwent subpectoral ICD implantation and DFT testing under GA. Continuous measurements of the absolute changes in the concentrations of oxyhaemoglobin (HbO2), de-oxyhaemoglobin (Hb) and cytochrome c oxidase copper moiety (Cyt a,a3) were obtained using near infrared spectroscopy (NIRS). MABP was monitored simultaneously. All episodes of induced VF (n = 26; duration 13.1±9.7 sec; cycle length 230.2±20.8 msec) and VT (n = 2; duration 15±2.8 sec; cycle length 320±11.3 msec) resulted in a decrease in MABP (23.9±7.5 mmHg, p<0.05) and HbO2 (-4.2±1.7 μmol/L; p<0.05), an increase in Hb (2.7±1.4 μmol/L) and no change in Cyt a,a3 redox status (0.09±0.30 μmol/L). This is the first prospective trial to perform neuromonitoring with NIRS during DFT testing. Our results suggest that oxygen availability is maintained at the cellular intra-mitochondrial level, despite a reduction in oxygen delivery to the cerebral cortex during short episodes of induced VF and VT.
