Abstract
Nokardiyoz, oldukça nadir görülen ve özellikle immünsüpresif olgularda ortaya çıkan fırsatçı bir enfeksiyondur.Ancak enfekte olan olguların yaklaşık üçte birinin immün sisteminin normal olduğu bilinmektedir.Nokardiyozun klinik tablosu çok geniş bir yelpazede oluşmakta ve bu nedenle diğer enfeksiyonlar hatta maligniteler ile bile karışabilmektedir.Patognomonik bulgusu, semptom ve radyolojik özellikleri olmayan bu hastalığın tanısı bazen güç olabilmektedir.Herhangi bir immünsüpresif hastalık ya da tedavi öyküsü olmayan olgumuz, nefes darlığı, halsizlik ve ateş yakınmaları ile başvurdu.Kliniğimize pnömoni ve parapnömonik efüzyon ön tanıları ile yatırıldı.Başlangıçta non-spesifik antibiyotik tedaviye yanıt vermesine rağmen, tekrarlayan pnömoni, beraberinde sağ kolda vena sefalica ve vena basilicada trombüs ile seyreden pulmoner nokardiyazis olgusu, nadir görülmesi ve tanı koymadaki zorluklar nedeni ile sunulmuştur.
