77 Validation of Transcutaneous Bilirubin Measurement Using Bilicheck™ In Premature Infants

To validate a transcutaneous bilirubin measurement using BiliCheck™ in premature infants. A total of 35 premature infants between 24–36 weeks gestation were studied. BiliCheck™ measurements were obtained with one hour of obtaining serum bilirubin levels using heel pricks. The BiliCheck™ measurements were obtained by a single investigator from the forehead skin. BiliCheck™ readings were within 10% difference than heel prick sample in infants between 30–36 weeks of gestation. BiliCheck™ readings were higher than serum samples. The difference was 18% in babies between 27–29 weeks of gestation. In infants <27 weeks of gestation, BiliCheck™ reading tended to correlate poorly with the serum bilirubin levels. BiliCheck™ is a useful, easy and non-invasive screening test for hyperbilirubinemia in premature infants >27 weeks of gestation. A serum confirmation is needed prior to initiation of therapy. BiliCheck™ reading correlated poorly in infants <27 weeks of gestation, which could be either due to premature skin properties or other factors i.e. high incubator humidity. These need to be investigated further.
