Il y a en Belgique environ une trentaine de projets de pratiques alternatives sur le terrain de la santé. Ceux-ci présentent un large éventail d’orientations différentes, mais la grande majorité sont le fait d’équipes de travailleurs de la santé. Ces projets ont toujours un caractère privé. Sans procéder à une quelconque évaluation de ces projets, l’auteur se pose un certain nombre de questions : n’est-on pas en présence de tentatives de modernisation du pouvoir médical ? Parvient-on, comme on l’espère, à redonner vraiment du pouvoir aux patients ? N’y a-t-il pas une tendance secrète à la médicalisation en véhiculant une définition extensive du concept de santé jusqu’à lui inclure le bien-être social ? Quelles sont les conditions d’une véritable participation ? Sur ce dernier point, ne conviendrait-il pas d’inverser la tendance des centres à rechercher une participation de la population en faisant en sorte que ce soit plutôt le centre qui offre sa participation à d’autres initiatives de la population, de groupements, d’organisations et d’institutions ? Une telle orientation permettrait de rendre à la santé ses limites et sa place dans la vie sociale, au lieu d’englober la vie sociale dans la santé. Des exemples illustrent cette hypothèse de travail. L’auteur s’interroge enfin sur les rapports qu’entretiennent la santé et la politique, le travail médical et le militantisme. Les équipes de santé doivent sortir de leur isolement tant au niveau de la pratique — par une collaboration avec d’autres groupes et organisations — qu’au niveau idéologique.
