We present a case of a 60-year-old male with a past medical history of alpha-1-antitrypsin deficiency with lung transplantation in 2010, who was maintained on immunosuppressive agents, and was admitted to the hospital for neurological symptoms. Magnetic resonance imaging (MRI) of the brain revealed a ring-enhancing lesion. Microscopic evaluation of the abscess revealed fungus, and cultures grew Cladophialophora bantiana ( C. bantiana ).
