Abstract
Doel Door middel van literatuuronderzoek en pilotonderzoek komen tot een objectief meetprotocol voor het meten van de (mogelijke) effecten van de McKenzie-methode in relatie tot pijn, Range Of Motion (ROM) en de functionaliteit van de lage rug in Algemeen Dagelijks Leven (ADL) / Health Related Quality of Life( HRQoL) bij patienten met lage rugklachten. Onderzoeksvragen o Wat zijn de (mogelijke) effecten van de McKenzie-methode in relatie tot pijn, ROM en de functionaliteit van de lage rug in ADL / HRQoL bij patienten met lage rugklachten? o Welke meetinstrumenten voor pijn, ROM en de functionaliteit van de lage rug in ADL / HRQoL zijn valide, betrouwbaar, responsief en bruikbaar voor het opstellen van een onderzoeksprotocol naar de (mogelijke) effecten van de McKenzie-methode? o Hoe ziet een pilot-onderzoek naar de praktische toepasbaarheid van de meetinstrumenten pijn, ROM en de functionaliteit van de lage rug in ADL / HRQoL binnen Fysio Group Limburg eruit, welke geevalueerd worden door middel van enquetes gelet op de bruikbaarheid, hanteerbaarheid en tijd door patienten en fysiotherapeuten om tot een meetprotocol te komen? Methode Door middel van literatuuronderzoek is gezocht naar valide, betrouwbare, responsieve en bruikbare meetinstrumenten voor pijn, ROM en ADL / HRQoL. Er is gezocht in Cinahl, Cochrane Library, Doconline, Google, Medline, Pedro, Pubmed en pijn.com. De gevonden artikelen zijn beoordeeld met behulp van Cochranelijsten. Daarna is data-extractie en data-analyse op de artikelen toegepast. Uit de artikelen kwamen een aantal meetinstrumenten naar voren welke getest zijn in de praktijk met behulp van een pilot-onderzoek en werden geevalueerd door middel van enquetes. Deze werden ingevuld door de fysiotherapeuten en patienten. Aan de hand van het literatuuronderzoek en het pilot-onderzoek is een meetprotocol ontwikkeld. Resultaten Aan de hand van het literatuuronderzoek kwamen verschillende meetinstrumenten naar voren voor het meten van pijn, ROM en ADL / HRQoL. Voor de pijn waren dit de Visual Analog Scale, de Numeric Pain Intensity Scale, de Verbal Numeric Rating Scale en de Verbal Rating Scale. Voor het meten van de Range of Motion kwam de Modified-Schobertest naar voren. Voor de functionaliteit van de lage rug in Algemeen Dagelijks Leven / Health Related Quality of Life kwamen de Quebec Back Pain Disability Scale, de Roland Disability Questionnaire, de Patient Specifieke Klachten en de Short Form-36 naar voren. Deze meetinstrumenten zijn tijdens het pilot-onderzoek getest. Conclusie Een meetprotocol voor het vaststellen van de (mogelijke) effecten van de McKenzie-methode bestaat uit de Verbal Numeric Rating Scale voor het meten van de pijn. Voor het meten van de Range Of Motion wordt gebruik gemaakt van de Modified-Schobertest en voor het vaststellen van de functionaliteit van de lage rug in Algemeen Dagelijks Leven / Health Related Quality of Life wordt de Quebec Back Pain Disability Scale gehanteerd. Summary Objective The development of an objective research protocol for measuring the (possible) impact of the McKenzie method in relation to pain, Range of Motion (ROM) and the functionality of the lower back in Active Daily Live (ADL) / Health Related Quality of Life (HRQoL) of patients with lower back complaints by means of a literature research and pilot testing. Research Questions o What are the (possible) effects of the McKenzie method in relation to pain, ROM and the functionality of the back in ADL / HRQoL of patients with lower back complaints? o Which measuring tools for pain, ROM and the functionality of the lower back in ADL / HRQoL are valid, reliable, responsive and useful for the development of a research protocol for measuring the (possible) impact of the McKenzie method? o What does a pilot-research, about the applicability of measurement tools for pain, ROM and the functionality of the lower back in ADL / HRQoL in Fysio Group Limburg, which will be evaluated by means of inquiries regarding the usability, handiness, time and costs for patients and physiotherapists to reach a measurement protocol, look like? Method By means of a literature research for valid, reliable, responsive and useful measuring instruments the following have been inspected: Cinahl, Cochrane Library, Doconline, Google, Medline, Pedro, Pubmed and pijn.com for valid. The articles have been assessed using Cochrane checklists. Afterwards date-extraction and date-analysis has been applied on the articles. The measuring tools mentioned in the articles were tested in practice during a pilot research. Based on the literature research and pilot research a measuring protocol has been developed. Results As a result of the literature research a number of measuring tools are suitable for measuring pain, ROM and ADL / HRQoL. Often there was insufficient information on the validity, reliability or responsiveness. The literature research resulted in the four following measuring tools for measuring pain: the Visual Analog Scale, the Numeric Pain Intensity Scale, the Verbal Numeric Rating Scale and the Verbal Rating Scale. For measuring the Range Of Motion the literature research proved Modified-Schobertest to be the most suitable. For measuring the functionality during ADL / HRQoL the following measuring tools are suitable: the Quebec Back Pain Disability Scale, the Roland Disability Questionnaire, the Patient Specific Complaints and the Short Form-36. These measuring tools have been tested during pilot research. Conclusion A measuring tool for determining the (possible) impact of the McKenzie-method consists of the Verbal Numeric Rating Scale for measuring pain. For measuring the Range Of Motion the Modified-Schobertest is used and for determining the functionality of the low back during ADL / HRQoL of the Quebec Back Pain Disability Scale is used.
