Les phénomènes de clivages d’équipes dans les institutions d’accompagnement des personnes du secteur sanitaire ou social sont une donnée quasi inhérente à ces organisations. Le travail centré sur la relation d’aide à l’usager ainsi que l’interdépendance des équipes à l’accomplissement de leur mission produisent un mode de défense groupal aux ramifications complexes et aux effets délétères. Le clivage s’inscrit dans un scénario d’exclusion et une logique rivalitaire entre collègues. La souffrance humaine que génère ce phénomène ainsi que les conséquences négatives pour la prise en charge des usagers restent insuffisamment explorées. En ce sens, nous proposons un apport clinique construit à partir de l’expression des professionnels lors de réunions d’équipe et de séances d’analyses des pratiques. Ces différents contextes de travail auprès d’équipe éducatives, paramédicales et de direction nous ont permis de décliner le clivage institutionnel comme une frontière tout en développant une typologie entre clivages stables/organisés vs instables/désorganisés. À partir de nos observations des souffrances et des évolutions probables des clivages institutionnels, nous proposerons des pistes d’intervention sous la forme de dispositifs institutionnels.
