Unrelated Hematopoietic Stem Cell (HSC) Transplant Outcomes. Results From the Argentine Registry

Abstract

Background: Hematopoietic stem cell transplant with unrelated donor is an option for patients (pts) with malignant or non-malignant diseases without a related histoidentical family donor. In 2003 the Argentine HSC Donors Registry has been created to offer a solution for these patients. In 2008 the first transplant for an argentine patient with an argentine donor was performed. In 2014 the Argentine Registry started HLA typing in high resolution and significantly increased the recruitment of donors impacting in a positive way in the amount of HSC collection as from 2015. Materials and methods: forty-three patients have been included in this study that required blood stem cell transplantation with unrelated argentine donor during 2015-2016, of which 30 were transplanted in Argentine Transplant Centers (TC) and 13 in international TC (USA, Germany, Norway, United Kingdom, Portugal, Spain, Canada, Italy, France and Uruguay). For the evaluation we included, the source of HSC collection, age, diagnosis, time of search, and overall survival. The median age of the argentine pts was 15 years old and 47 years old for international pts. The HSC source was bone marrow in 13 pts (30%) and peripheral blood in 30 pts (70%). The diagnosis was: Acute Lymphoblastic Leukemia 13 (30%), Acute Myeloblastic Leukemia 11 (26%), Severe Aplastic Anemia 6 (14%), Myelodisplastic Syndrome 4 (9%), Non-Hodgkin lymphoma 2 (5%), Fanconi Anemia 2 (5%), Chronic lymphocytic leukemia 1 (2%), Wiskott Aldrich Syndrome 1 (2%), Hemophagocytic Syndrome 1 (2%), Mayor Thalasemia 1 (2%), Primary Immunodeficiency 1 (2%). Considering the patient/donor high resolution HLA compatibility in HLA-A, B, C, DRB1 and DQB1, 25 (53%) where 10/10, 17 (39%) where 9/10 and 1 (2%) was 8/10. Results: The median time from a search request to the transplant with an unrelated argentine donor for argentine pts was of 3 months and 18 days and for international pts was 3 months and 12 days. The median time of response for donor search request and shipping the HLA sample was in both cases 11 days. The median time between the HLA donors report and the HSC collection was of 85 for argentine pts and 84 for international pts. At one year the overall survival for the group from the transplant was 79.4%, for adult pts 42.9% and for pediatric pts 77.7%. The overall survival at one year for pts according to the HLA compatibility patient/donor was 89.5% for 10/10 and 63.8% for 9/10. Conclusions: This data shows the importance of having an Unrelated Donors Registry in our country. The times of search and survival were similar to those reported from international registries. These are preliminary data, needing more time and patients to achieve a more accurate analysis.
