Eficacia de un programa de ejercicio físico activo y estimulación táctil en bebés de riesgo neurológico y sus efectos en el desarrollo cognitivo y psicomotor y el estrés parental

Abstract
Objetivo Valorar el efecto del masaje infantil y ejercicio fisico facilitado sobre el desarrollo mental y psicomotor de ninos de alto riesgo neurologico, asi como su influencia sobre la regulacion del sueno de los ninos y el estres de sus madres. Metodo Estudio cuasi-experimental longitudinal con ninos de riesgo neurologico, de los 4 a los 18 meses de edad corregida, divididos en grupo experimental (n=44) y grupo control (n=36) pertenecientes a los Equipos de Atencion Temprana de Murcia. Se realizo un programa de entrenamiento a los padres en ejercicio fisico y masaje infantil, dirigidos por un fisioterapeuta. Esta intervencion consta de dos fases: la primera con una sesion semanal durante cinco semanas consecutivas y la segunda con cuatro sesiones trimestrales. A los padres se les recomendaba repetirlo a sus hijos una vez al dia en el domicilio. A todos los ninos se les evaluo el nivel de desarrollo psicomotor y cognitivo al inicio del programa (4 meses de edad corregida), a los 12 y a los 18 meses de edad corregida con la Bayley Scales of Infant Development-II y Alberta Infant Motor Scale. Los padres de estos ninos cumplimentaron un cuestionario relacionado con el sueno nocturno de sus hijos, al inicio, a los 12 meses y a los 18 meses Ademas, el estres de los padres del grupo experimental se evaluo al inicio y tras las primeras cinco semanas de la intervencion mediante el cuestionario Parenting Stress Index. Resultados El nivel de desarrollo motor, medido con Alberta Infant Motor Scale, mejoro en el grupo experimental respecto al grupo control a los 12 meses, obteniendo aumentos significativos tanto en los centiles (p=0,01), como en las puntuaciones directas (p=0,01) y en la subescala de prono (p=0,004). En la subescala de sedestacion las mejoras fueron marginalmente significativas (p= 0,06). Tras las primeras cinco semanas de intervencion encontramos un aumento significativo en las horas (p=0,001) y calidad de sueno nocturno (p=0,001) de los ninos del grupo experimental, asi como una disminucion en el nivel de estres de las madres p=0,01). La adherencia de las madres a la intervencion domiciliaria fue mayor al inicio, cuando las sesiones de entrenamiento eran semanales. Conclusiones El masaje infantil y el ejercicio fisico facilitado por los padres a los ninos de alto riesgo parece mejorar el desarrollo psicomotor a corto plazo y medio plazo. Este tipo de intervencion parece influir favorablemente tambien en la regulacion del sueno nocturno de los ninos y en el nivel de estres de sus padres. Objective To assess the effect of infant massage and active physical exercise on mental and psychomotor evolutionary development in children who are at high neurological risk. Influence of this intervention on the regulation of the children's sleep patterns and their mother's stress levels was evaluated as well. Method A longitudinal quasi-experimental study with children at neurological risk, from 4 to 18 months corrected age, who are overseen by the Early Support Teams of Murcia. The children were divided into an experimental group (n = 44) and a control group (n = 36). A training programme of physical exercise and infant massage was presented to the parents, conducted by a physiotherapist. This intervention consisted of two phases: the first of weekly sessions for five consecutive weeks and the second of four quarterly sessions. Parents were encouraged to repeat the programme daily with their children at home. The level of initial development of all children was assessed during the first four months and at 12 and 18 months corrected age, using the Bayley Scales of Infant Development-II and the Alberta Infant Motor Scale. The parents of the children from both groups completed a questionnaire to assess the effect of the programme on the nocturnal sleep of their children, at the beginning of the programme and at 12 months and 18 months. After the first 5 weeks of intervention, the sleep of the experimental group was evaluated. Also, the stress levels of the parents in the experimental group were assessed at both the beginning and after the first 5 weeks of intervention, using the Parenting Stress Index test. Results The level of motor development, measured with the Alberta Infant Motor Scale increased in the experimental group vs control at 12 months improved significantly overall (p=0.01), as seen in the percentile score as well as the direct scores for example, in the total score (p=0.01), and the prone subscale (p =0.004). There was a marginally significant improvement in the sitting subscale (p=0.06). After the first five weeks of intervention we found significant improvements in quantity (p = 0.001) and quality (p = 0.001) of nocturne sleep in experimental group, as well as a reduction in the stress levels of the mothers (p = 0.01). The adherence of the mothers, to home-based intervention is greater at the beginning, when the training sessions are weekly. Conclusions Infant massage and active physical exercise employed by the parents of the high-risk children seems to improve, both the short-term and long-term psychomotor development. These interventions seem to improve also the nocturne sleep regulation of the children and reduce the stress of their mothers.
