Abstract
This chapter reviews isolated atrial septal defect (ASD). Diagnosis of an isolated ASD in the neonatal period is exceedingly rare unless it is an incidental finding on echocardiogram performed for another indication. Two-dimensional echocardiography combined with color Doppler aims to define the position, including the length of the septal rims, size, and hemodynamic effect of an ASD. Although it is extremely rare for an ASD to need treatment in the neonatal period, it is important to be aware of treatment strategies in later childhood and beyond. The aim of closure of an ASD is first to ameloriate symptoms and secondly to avoid the potential risk of causing irreversible pulmonary hypertension. Surgical repair is the treatment of choice for ostium primum, sinus venosus, and coronary sinus ASDs. Closure of sinus venosus ASDs is a challenge as a consequence of the presence of anomalous pulmonary veins.
