Abstract
Background: Hemophagocytic lymphohistiocytosis (HLH) is a rare immune-regulatory disorder and allogeneic hematopoietic stem cell transplantation (HSCT) is currently the only curative option. Recent data suggests that reduced intensity conditioning (RIC) regimens reduces the acute transplant mortality compared to myeloablative regimens (MAC). However, it remains unclear if the increased risk for graft failure (GF) and HLH recurrence in the setting of mixed donor chimerism in RIC patients offsets the benefits of decreased acute toxicity.
