Et ailleurs…?Protéinurie, filtration glomérulaire et insuffisance rénale aiguë: message pour la pratique La question Environ 3 à 7% des patients qui entrent à l'hôpital souffrent d'insuffisance rénale aiguë (IRA).Quelle est l'importance du taux de filtration glomérulaire et de la protéinurie antérieure à l'épisode d'IRA dans la survenue de celui-ci et dans l'évolution de l'IRA?Ces questions sont capitales notamment pour la reconnaissance des patients à risque et pour éviter par exemple l'administration de médicaments néphrotoxiques (AINS, produits de contrastes entre autres). La méthodeUne étudedecohorte comprenant environ920 000 personnes (!) a été réalisée au Canada entre 2002 et 2007.Les patients ont été divisés en 4 groupes selon leur eTFG (>60 ml/min/1,73 m 2 , 59,9-45, 44,9-30, 29,9-15).Les patients avec un eTGF <15 n'ont pas été inclus dans l'étude.Pour la protéinurie, 3 groupes ont été constitués: aucune, modérée, forte. Les résultatsDurant un suivi médian de 35 mois, 6520 patients ont été admis avec une IRA et 615 avec une IRA nécessitant un traitement dialytique.Le résultat essentiel est l'importance majeure de la protéinurie de départ: par ex.pour les patients avec un eTFG >60 (presque normal), une protéinurie forte augmente le risque d'IRA de 4,4 fois par rapport aux patients avec une protéinurie nulle.Pour toutes les valeurs de eTFG, une protéinurie forte constitue un risque plus élevé d'IRA.
