INTRODUCTION AND AIMS: Cognitive impairment (CI) is common in patients with chronic kidney disease. In dialysis patients, cognitive impairment is associated with increased resource utilization, higher risk of hospitalization and all-cause morbidity. In patients with chronic disease, sleep disorders are more prevalent, poor sleep quality can negatively affect immunity, cognitive functions, and emotional manifestations. The aim of this study is identify the abnormalities of cognitive function and find its association with sleep quality in end stage renal disease (ESRD) patients undergoing routine hemodialysis METHODS: This is a cross sectional study that took place in four hemodialysis clinics in Jakarta, Indonesia. Patients which had undergone hemodialysis for more than 3 months were included in this study. Cognitive function was assessed using Montreal Cognitive Assessment (MoCA-Ina). Cognitive impairment was defined as MoCA score less than 26. Sleep quality was assessed using Pittsburg Sleep Quality Index (PSQI). Analysis was done using Mann-Whitney U Test. RESULTS: A total of 113 patients were included in this study, out of which 72 (63.7%) were male. The mean age of the patients was 53.2 (±12.5) years. The most common etiology of chronic kidney disease was due to hypertension (58.4%), followed by diabetes mellitus (29.2%). Cognitive impairment was present in 97 (85.8%) patients. The median scores of visuospatial, naming, attention, language, abstraction, delayed recall and orientation domains were 3 (0-5), 3 (0-3), 4 (0-6), 2 (0-3), 1 (0-2), 0 (0-5) and 6 (0-6) respectively. Cognitive impairment had significant association with sleep quality (P = 0.026). The median sleep quality score in patients with and without cognitive impairment were 12 (0 - 21) and 8.5 (2 - 16) respectively. CONCLUSIONS: More than half of the patients in our study have cognitive impairment. Cognitive impairment has significant association with sleep quality in hemodialysis patients. As physicians, we should try to modify the sleep quality in order to improve the cognitive function of these patients.
