Abstract
Most patients fail to achieve remission following an adequate first-line antidepressant treatment for a major depressive episode, and even after four consecutive trials, a third of patients have still not achieved remission. Since one or more monoamines are the primary target for each of these treatment strategies, it is hardly surprising that attempts to develop new antidepressants have expanded beyond monoamine targets. These initiatives are driven by emerging evidence to support alternative models of brain dysregulation in depression. One example involves the hypothesis that circadian rhythm disruption may result in depression [1]. This chapter will address the relationship between melatonin agonism and 5-HT2C receptor antagonism, circadian rhythm and mood regulation and agomelatine as a novel antidepressant.
