Malgré les succès récents qui ont permis de réduire l'incidence de la tuberculose, cette maladie demeure un important problème au Canada, en particulier chez les personnes nées à l'étranger et les populations autochtones.Le traitement de l'infection tuberculeuse latente (ITL) est l'un des éléments clés de la stratégie visant à lutter contre cette maladie.Le traitement classique consiste en la prise quotidienne d'isoniazide (INH) pendant neuf mois.Au cours des dernières années, des schémas thérapeutiques de plus courte durée ont été mis au point dans l'espoir d'accroître les taux d'acceptation et d'achèvement du traitement.Le plus court et le plus récent de ces traitements associe l'INH et la rifapentine administrés une fois par semaine pendant 12 semaines (3HP), habituellement en thérapie sous observation directe.Ce schéma a été approuvé par la Food and Drug Administration aux États-Unis, mais il n'est pas encore autorisé au Canada.Si l'on se fie aux études d'observation et essais randomisés contrôlés dont le nombre augmente rapidement, le schéma 3HP pendant 12 semaines semble avoir une efficacité comparable à celle du traitement de neuf mois par l'INH, en plus de présenter un profil d'événements indésirables favorable et d'offrir de meilleurs taux d'achèvement du traitement.Bien que le taux d'acceptation du traitement, ou le rôle du traitement auto-administré ou la rentabilité du schéma thérapeutique dans le contexte canadien, restent à déterminer, le 3HP semble donc offrir un traitement de rechange prometteur aux traitements existants de l'ITL.
