Abstract
Anaphylaxis is a clinical diagnosis that describes a complex and varied clinical and physiologic phenotype. Triggers, timing and reaction severity can vary, even within the same individual. There is an appreciation now that a number of cell types beyond mast cells are involved, including basophils, macrophages and neutrophils. Pathways involving IgG-mediated anaphylaxis and complement activation have been recognized in addition to IgE-mediated mechanisms. Severe anaphylaxis may be due to the additive or synergistic actions of multiple mediators converging in a final common pathway. Alternatively, severe anaphylaxis may occur only when certain mediators are involved, for example mediators that can profoundly affect breathing or the cardiovascular system. Finding biomarkers that reflect severity of anaphylaxis for all or at least most causes would be very beneficial to guide treatment, risk assessment and prevention strategies.
