Abstract
Complete resection of pancreatic carcinoma is the only possibility of curative therapy. Hence, the fundamental aim of all investigatory procedures for preoperative staging and individualized therapy planning is the differentiation of those patients with either resectable, doubtful resectable or unresectable tumors (Figure 3.1). Surgical treatment of these patients obtained considerable advances during the last few years and perioperative morbidity and as a consequence mortality decreased in the so-called high-volume centers. Despite these achievements in early results, approximately 80% of patients show unresectable tumor stage at the time of making the diagnosis. Furthermore and consistingly with the small number of long-term survivors, early local tumor recurrence and/or predominantly hepatic metastatic spread due to lymphatic and perineural tumor cell dissemination must be mentioned. In this situation, surgical therapy has its status in palliation and facilitation of best possible life quality – always dependent on morbidity and patient’s general health condition.
