Abstract
L’hypothermie thérapeutique est une norme de soins pour les nouveau-nés de 36 semaines d’âge gestationnel (AG) ou plus qui sont atteints d’encéphalopathie hypoxo-ischémique modérée à grave. Puisque certaines études incluaient des nouveau-nés de 35 semaines d’AG, il faut envisager l’hypothermie s’ils satisfont aux autres critères. Le refroidissement des nouveau-nés de moins de 35 semaines d’AG n’est pas recommandé. Dans les hôpitaux communautaires, il faut amorcer le refroidissement passif rapidement, en consultation avec un néonatologiste d’un centre de soins tertiaires, tout en surveillant de près la température du nouveau-né. D’après les données exemplaires, la mesure optimale consiste à maintenir la température centrale entre 33 °C et 34 °C pendant 72 heures, puis à entamer une période de réchauffement de six à 12 heures. Il faut utiliser des antiépileptiques en présence de convulsions cliniques ou électrographiques. Le maintien des électrolytes et du glucose sériques dans les plages normales et l’évitement de l’hypocapnie, de l’hypercapnie et de l’hyperoxie sont d’importants traitements d’appoint. Il est conseillé de procéder à une imagerie par résonance magnétique (IRM) du cerveau peu après le réchauffement et, lorsque les observations antérieures ne correspondent pas au portrait clinique, de reprendre l’IRM après l’âge de dix jours. Un suivi neurodéveloppemental multidisciplinaire est recommandé.
