Kardiales Remodeling und Veränderung der diastolischen Funktion nach renaler Denervierung bei Patienten mit therapierefraktärem Hypertonus

Abstract
Zielsetzung: In der jüngeren Vergangenheit, wurde die Katheter-Ablation (renale Denervierung, RDN) als vielversprechende Therapieoption bei Patienten mit therapierefraktärem Hypertonus untersucht. Ziel dieser Studie war es die Auswirkungen auf die kardiale Masse und die diastolische Funktion mittels Kardio-MRT (CMR) zu quantifizieren. Material und Methoden: 18 Patienten mit therapierefraktärem Hypertonus wurden mittels renaler Denervation behandelt. Für die quantitative CMR wurde eine EKG-getriggerte steady-state free precession (SSFP) Cine Sequenz vor und 12 Monate nach RDN durchgeführt. Die quantitative Analyse umfasste das enddiastolische Volumen (EDV), das endsystolische Volumen (ESV), das Schlagvolumen (SV), die Ejeketionsfraktion (EF), die linksventrikuläre myokardiale Masse (LVMM), die enddiastolische Spitzenfüllungsrate (EPFR) und die Vorhofspitzenfüllungsrate (APFR). Zur Beurteilung der diastolischen Funktion wurde das Verhältnis von EPFR und APFR gebildet. Sämtliche Parameter wurden auf die Körperoberfläche indiziert. Die Auswertung erfolgte durch zwei unabhängige Untersucher. CMRtools wurde zur Analyse verwendet. Ergebnisse: Bei Patienten mit therapierefraktärem Hypertonus führte die renale Denervierung innerhalb von 12 Monaten zu einer signifikanten Abnahme der linksventrikulären Myokardmasse (LVMMi) (80 ± 22 g/m2 vs. 73 ± 21 g/m2, p < 0.01). Das EDV (78 ± 15 ml/m2 vs. 80 ± 21 ml/m2), das ESV (45 ± 8 ml/m2 vs. 46 ± 13 ml/m2), das SV (92 ± 20 ml vs. 96 ± 30 ml) und die EF (59 ± 11% vs. 60 ± 13%) zeigten keine signifikaten Veränderungen. Das EPFR/APFR-Ratio zeigte eine tendenzielle Erhöhung (0.907 ± 0.379 vs. 1.064 ± 0.509, p < 0.06) ohne statistische Signifikanz zu erreichen. Schlussfolgerungen: RDN führt zu einer Abnahme der linksventrikulären Myokardmasse 12 Monate nach Intervention. Eine Verbesserung der LV-Funktion konnte nicht nachgewiesen werden. Es zeigt sich jedoch eine positive Auswirkung auf die diastolische Funktion.
