In this issue of Neurology ®, Sperling et al.1 describe neuropsychological evaluations associated with their prior industry-sponsored, double-blind, sham-controlled, randomized clinical trial of MRI-guided focused ultrasound (MRIgFUS) ablation of the ventral intermediate nucleus (Vim) for tremor-dominant Parkinson disease (PD).2 MRI thermography is used to monitor focused sonications with an ultrasound system to ablate small areas. It is approved by the Food and Drug Administration for uterine fibroids and for ablation of the Vim thalamus for essential tremor. Although noninvasive, the computer algorithms used require the patient's head to be shaved and kept still in a stereotactic frame. Since the lesion is made in real time, intraprocedural neurologic testing can be performed.
