Abstract
Background: The renin-angiotensin-aldosterone system plays a crucial role in blood pressure (BP) regulation. The aim of this study was to investigate the relationship between the aldosterone-to-renin ratio (ARR) and several BP indices in a representative sample of healthy adults from the general population. Methods: Data are based on a population-based study among young and healthy adults without antihypertensive treatment. Conventional and ambulatory BP measurements were obtained using validated devices. Direct renin concentration (DRC) and plasma aldosterone concentration (PAC) were measured from frozen plasma samples. The association of log-transformed ARR with BP indices and hypertension was computed using multivariable linear and logistic regression models. ... Results: This analysis included 1,350 individuals. Median age (IQR) was 37 (31–40) years and 760 (56%) were female. Median (interquartile range) ARR, DRC and PAC were 13.8 (8.7; 22.8), 7.2 (4.4; 11.0) and 94 (68; 134) ng/l. All BP indices increased across sex-specific ARR quartiles. In multivariable adjusted regression models, there were independent linear relationships between ARR and all BP indices. Per 1-unit increase in ARR, the β–coefficients (95% confidence interval) for systolic conventional, 24-h, daytime and nighttime BP were 1.47 (0.65; 2.28), p=0.0005; 2.24 (1.50; 2.98), p<0.0001; 2.05 (1.28; 2.82), p<0.0001; and 2.7 (1.92; 3.51), p<0.0001, respectively. Results of the relationships between ARR and different types of hypertension are shown in Table 1.
