Abstract
Введение врожденные пороки развития легких в структуре младенческой смертности и заболеваемости составляют 20,1% [1].среди всех пороков развития легких 50-75%, по с.п.ДосМАГАМбеТоВ, б.к.ДженАЛАеВ, б.н.бисАЛиеВ, А.б.ТусупкАЛиеВ, А. бАТЫроВ Врожденная кИстозная МальфорМаЦИя легкИХ у ноВорожденнЫХ западно-казахстанский государственный медицинский университет имени марата оспанова, г. актобе, республика казахстан Цель.провести оценку диагностических и лечебных мероприятий при редком врожденном пороке развития легких у новорожденного ребенка.Материал и методы.приведен клинический случай врожденной кистозной мальформации легкого у новорожденного ребенка.описана клиническая симптоматика, диагностика, интраоперационная тактика, послеоперационное ведение.Результаты. анализ истории болезни новорожденного ребенка с врожденным пороком развития легкого свидетельствует о проявлении респираторного дистресс-синдрома с первых часов жизни ребенка.компьютерная томография легких не только позволила подтвердить характер изменений в левом легком, но и уточнить локализацию поражения, состояние непораженных участков легких, степень смещения средостения, провести дифференциальную диагностику с другими врожденными пороками развития легких.удаление пораженного участка левого легкого привело к уменьшению дыхательной недостаточности, устранению гемодинамических нарушений, легочной гипертензии.Заключение. врожденная кистозная мальформация легких является относительно редким пороком развития.если же порок развития легкого не диагностирован антенатально, то диагностика ее с первых дней жизни ребенка представляет непростую задачу.проявление респираторного дистресс-синдрома у ребенка с первых дней жизни, правильная интерпретация обзорной рентгенограммы грудной клетки, проведение компьютерной томографии легких позволяют своевременно и правильно поставить диагноз.удаление порочной доли легкого в неонатальном периоде является патогенетически обоснованным методом лечения.в послеоперационном периоде дети требуют длительного диспансерного наблюдения.Ключевые слова: врожденный порок развития легких, кистозная мальформация легкого, кистозная гипоплазия, респираторный дистресс-синдром, пневмоторакс, диагностика, хирургическое лечение Objective.To assess diagnostic and therapeutic measures in rare malformation of the lungs of a newborn.Methods.The clinical case of congenital cystic malformation of the lungs in a newborn is given.clinical symptoms, diagnostics, intraoperative tactics, and postoperative management are described.Results.The analysis of case history of a newborn with congenital lung malformation denotes the manifestation of respiratory distress syndrome in the first hours of a child's life.computed tomography (cT) of the lungs allowed not only to confirm the nature of the changes in the left lung, but also to clarify the localization of the lesion, the condition of not affected areas of the lung, the degree of mediastinum displacement, allowed to make a differential diagnosis with other congenital lung malformations.removing the affected area of the left lung resulted in a decrease of respiratory failure, elimination of hemodynamic disorders and pulmonary hypertension.Conclusions.congenital cystic lung malformation is a rather rare malformation.but if malformation of the lung is not diagnosed during antenatal period, then its diagnostics from the first days of a child's life could be not a simple task.The manifestation of respiratory distress syndrome in a child with the first days of life, the correct interpretation of the plan X-ray pictures of the chest, cT of the lungs allow to put diagnosis correctly and in proper time.The removal of a faulty lobe of the lung in the neonatal period is a pathogenetically justified method of treatment.in the postoperative period children require long-term follow-up.
