Abstract
Резюме.Цель -выявление и оценка факторов, влияющих на исход тотального протезирования коленного сустава (ТЭКС), на основании изучения и анализа ранних результатов оперативного лечения гонартроза в условиях клиники травматологии, ортопедии и военно-полевой хирургии на базе Витебской областной клинической больницы.Материал и методы.В исследование включено 134 пациента, которым было выполнено ТЭКС по поводу гонартроза 3-4 стадии.Пациентов с наличием вальгусной/варусной деформации коленного сустава более 10° мы выделили в отдельную группу.Пациенты обследованы по анкетам Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) и Self-administered patient satisfaction scale.Средний срок наблюдения составил 8,1 (+/-) месяца.Результаты.Общий средний балл по WOMAC до операции составил 26,8±5,8, после операции 73,2±7,8.Постоянный болевой синдром отметили 7 (5,2%) пациентов, периодические боли в области сустава беспокоят 23 (17,2%).По результатам нашего исследования, с использованием Self-administered patient satisfaction scale для оценки исходов оперативного лечения, 83,8% пациентов оценило ближайший результат операции как «отличный» либо «хороший», 12,5% как «удовлетворительный» и 3,8% как «плохой».Межгрупповых различий при анализе результатов исследования выявлено не было.Заключение.ТЭКС позволяет добиться положительных результатов лечения у большинства пациентов.Степень предоперационной деформации оси конечности не оказывает существенного влияния на удовлетворенность пациентов результатами оперативного лечения в раннем послеоперационном периоде.Резерв улучшения результатов ТЭКС мы видим как в совершенствовании периоперационного обеспечения, так и в повышении до-и послеоперационной информированности пациентов.
