Abstract
Le présent point de pratique porte sur les enfants âgés de 90 jours à 17 ans qui viennent de recevoir un diagnostic de thrombopénie immune (TPI) primaire classique. Les recommandations à jour sur la prise en charge et l’information tirée d’études récentes sont résumées afin de limiter la variabilité des pratiques chez les dispensateurs de soins et d’améliorer les soins axés sur les patients. Le maintien en observation sans pharmacothérapie fait partie des possibilités de prise en charge initiale des jeunes patients atteints de TPI qui ont des ecchymoses, des pétéchies ou une épistaxis légère et occasionnelle sans incidence sur leur vie quotidienne. En cas de traitement actif, les corticoïdes et les immunoglobulines par voie intraveineuse sont à envisager. Il est encore recommandé d’hospitaliser et de traiter les enfants ayant des saignements modérés ou graves. La prise de décision partagée peut améliorer les soins axés sur le patient et garantir que les familles comprennent pleinement les possibilités de prise en charge qui leur sont offertes.
