Prise en charge nutritionnelle et gestion du surpoids dans le diabète de type 1

Abstract
Les patients avec un diabète de type 1 (DT1) sous insulinothérapie intensive se doivent de pratiquer le calcul des glucides (CG), mais cette tâche ajoute un fardeau à la gestion de la maladie. De plus, cette méthode contribue à une approche nutritionnelle glucocentrique non axée sur la qualité alimentaire. En effet, un questionnaire nous a démontré que les patients avec un DT1 avaient certaines difficultés dans son application et des études antérieures ont prouvé que ces patients avaient une faible qualité alimentaire. Parallèlement, le profil des patients DT1 a beaucoup changé, en particulier avec la progression des problèmes de surpoids et d’obésité qui contribuent à une prévalence de syndrome métabolique allant jusqu’à 45%, un statut associé à une majoration du risque cardiovasculaire (CV). En l’absence de recommandations spécifiques pour ce groupe à ce jour, nous avons démontré qu’une intervention nutritionnelle axée sur la qualité alimentaire (diète faible en gras et méditerranéenne) était faisable et menait à une amélioration du profil cardiométabolique. L’absence de différences significatives sur le profil CV et la composition corporelle entre ces deux diètes suggère que l’individualisation de l’approche nutritionnelle serait nécessaire pour les patients avec un DT1 pour la réduction du risque CV. Des études supplémentaires sont cependant requises pour optimiser l’enseignement et changer l’approche glucocentrique afin de faire une promotion plus importante de la qualité nutritionnelle s’intégrant dans de saines habitudes de vie.
