Les préoccupations relatives aux ruptures de stock en médicaments antirétroviraux et leurs conséquences en termes de résistances biologiques aux traitements contre le VIH sont passées relativement inaperçues jusqu'à récemment. En effet, ces problèmes, qui prennent depuis peu une place plus importante au sein de politiques de santé mondiale, ont été mis à l'arrière-plan d'un agenda international porté depuis 2009 par une vision technicienne et volontariste, affichant l'horizon de l'éradication de la maladie et d’"un monde sans sida" pour 2030. L'échec des thérapies antirétrovirales affectant les pays à ressources limitées est maintenant mieux reconnu et même parfois présenté comme la « quatrième épidémie de VIH ». Alors qu'une expertise technique est nécessaire pour appréhender les problèmes ainsi posés, une perspective anthropologique plus large permet de les concevoir comme les effets indésirables de la généralisation des traitements antirétroviraux impliquant des dimensions intimes, biologiques et sociales entremêlées. En proposant une ethnographie des échecs du traitement en République centrafricaine, cet article examine les effets biologiques, individuels, sociaux et politiques d'une « insécurité pharmaceutique », qui constitue, dans de plus en plus de contextes africains, l'envers trop peu discuté des promesses biologiques et politiques générées par la massification des traitements antirétroviraux.
