This 156-page soft cover book is edited by Douglas Deporter who has been a full time faculty member of the Faculty of Dentistry at the University of Toronto since 1976. Deporter has published extensively on short and ultra-short implants and was coinventor of the Endopore implant system. The book is written by an international group that includes residents, private practice clinicians, and university professors. Chapter 1: Why Avoid Using Short Implants This chapter addresses the concerns most clinicians have with the use of nonstandard length implants. Implant success rates, crown-to-implant ratio, and crestal bone loss are addressed through literature reviews. The authors conclude that short implants are not necessarily inferior to standard-length dental implants. Chapter 2: The Performance of Short and Ultra-Short Implants The definition of short and ultra-short implants is stated in this chapter. The authors also explore the topics of implant macro design, implant surfaces, abutment interface, crown-to-implant ratio, implant splinting, osteotomy preparation and insertion torque, and how these factors impact the short- and long-term success of dental implants. The success rates of short and ultra-short implants are reviewed and compared with those of standard-length implants. The chapter concludes by stating that clinical experience and data are starting to challenge the dogma that short and ultra-short implants have unacceptable failure rates but concedes that many factors impact clinical outcome. Chapter 3: A Single Practitioner's 20-Year Experience With Short Implants This chapter is written by a periodontist in private practice and shares his 20-year well-documented experience using 6- and 8-mm long threaded implants, specifically the Straumann Tissue Level (STL) implants. The chapter has many clinical photographs and radiographs that showcase his experience with these implants and some of their indications and limitations. The author concludes his chapter by stating that the 8-mm STL implants appear to perform as well as standard-length STL implants but that the 6-mm STL implants have not done as well. Chapter 4: Using Short Implants for Overdenture Support Although the use of standard-length dental implants to support a mandibular overdenture has been well documented, this chapter presents the use of shorter dental implants for overdenture retention. The authors present guidelines for using short implants in the resorbed anterior mandible and describe both the surgical and prosthetic procedures. The procedures are well presented with radiographs and clinical photographs. Chapter 5: Threaded Implants in the Posterior Maxilla The authors of this chapter start by stating that placing dental implants in the posterior maxilla remains a challenge due to reduced bone quality and quantity, limited visibility, and increased interarch space. The authors continue the chapter discussing sinus elevation procedures and their associated complications and then discuss the use of short moderately rough threaded implants (MRTIs) in the posterior maxilla as an alternative to sinus elevation. Patient selection and treatment guidelines for short MRTI placement are reviewed along with clinical examples. Chapter 6: Threaded Implants in the Atrophic Posterior Mandible The focus of this chapter is on the use of short MRTIs for the atrophic posterior mandible. The chapter begins with the limitations of vertical ridge augmentation and then presents a case for the use of short MRTIs. Patient selection and treatment guidelines for the use of short MRTI in the atrophic posterior mandible are presented, and multiple radiographs and clinical photographs are used to demonstrate clinical cases. Chapter 7: Press-Fit Sintered Porous-Surface Implants The history and development of press-fit sintered porous-surface implants (SPSIs) are presented in this chapter. The author discusses clinical case selection for their use as well as the challenges with SPSIs. A good portion of the chapter discusses the use of SPSIs in conjunction with the osteotome sinus elevation technique and provides clinical guidelines for success treatment outcomes. Chapter 8: Plateau Root Form Implants The author of this chapter discusses the benefits of plateau root form (PRF) implants. Clinical indications and guidelines for the use of PRF implants are presented and demonstrated with radiographs and clinical photographs. Chapter 9: Ultra-Wide Threaded Implants for Molar Replacement Single implant placement in the molar sites often presents anatomical constraints related to reduced bone quality as well as the location of the mandibular neurovascular bundle or a pneumatized maxillary sinus. This chapter looks at short wide implants in these sites and provides treatment protocols and sample cases. Multiple clinical photographs and radiographs help illustrate the use of short wide implants in molar sites. The chapter concludes that immediate replacement of molars with a short wide implant is an important addition to the implant dentist's armamentarium but that a strict surgical protocol must be followed. Chapter 10: The Way Forward The book's final chapter is written by the editor, Douglas Deporter, who shares his thoughts on short dental implants and his vision for further research and their continued evolution in implant dentistry. The recently published “Short and Ultra-Short Implants” edited by Douglas Deporter challenges the dogma that short and ultra-short dental implants have an unacceptably high failure rate. The book is concise and well written and features many clinical photographs and radiographs demonstrating a multitude of clinical applications. The book's many authors attempt to make a case for the use of short and ultra-short dental implants with the limited data available. This work is also candid in discussing the limitations of these implants and strict case selection and guidelines necessary to insure success. Although the book will not sway a seasoned clinician in giving up predictable bone regeneration techniques in lieu of short implants, it does provide some insight for the addition of short and ultra-short implants to the clinicians' armamentarium.
