Abstract
Hypotheses: 1. Unilateral grade 2 sacroiliitis does not necessarily progress in psoriatic arthritis (PsA) 2. Ankylosing spondylitis (AS) with psoriasis and axial PsA are different phenotypes of the same disease Methods: Retrospective observational cohort study. Results: Progression of unilateral grade 2 sacroiliitis was seen in 52% of the PsA cohort, it was associated with: elevated ESR, short disease duration, young age and less degenerative disc disease. AS patients compared to axial PsA were more men, more likely to have HLA-B*27, had worse axial disease and milder peripheral arthritis. Concomitant psoriasis in AS did not influence the course of the disease. Discussion: Unilateral grade 2 sacroiliitis does not necessarily progress in PsA. The differences between axial PsA and AS, with and without psoriasis, were substantial, suggesting these are separate diseases with overlapping features. The presence of psoriasis in AS patients does not seem to affect the disease course.
