Selon l’approche des déterminants sociaux de la santé, la santé des populations, et les inégalités de santé qui sévissent en leur sein, sont en grande partie influencées par des facteurs sociaux, modifiables par les politiques publiques, qui se situent bien en amont de ces états de santé. Il en découlerait que les politiques publiques favorables à la santé devraient intervenir à ce niveau causalement fondamental. Cependant, la présence indéniable de déterminants sociaux n’implique pas que nous saisissions bien les mécanismes précis par lesquels ces déterminants se font les véhicules des états de santé. Les recherches sur les déterminants donnent lieu à des résultats tout au mieux probabilistes. Étant donné que la santé est un bien par rapport auquel les décideurs publics doivent se prémunir contre le risque et l’incertitude, des raisons existent pour qu’ils privilégient des approches plus traditionnelles à la santé, basée sur des chaînes causales plus courtes.
