A patient in his 60s with nonalcoholic steatohepatitis cirrhosis and type 2 diabetes mellitus was admitted to the hospital for ataxia. He reported a 2-month history of progressive constant vertigo and a 1-month history of progressive gait unsteadiness eventually requiring a wheelchair. On examination, he had downbeating nystagmus in all directions of gaze and hypermetric saccades. He had scanning dysarthria, dysmetria in all limbs, and an ataxic, wide-based gait. Blood pressure was normal. There was no history of antiseizure medications, chemotherapeutic agents, or substance abuse including alcohol, heroin, and opioids.
