Abstract
Un homme de 72 ans s’est présenté avec des symptômes de perte de champ visuel inférieur unilatéral, mais on a découvert qu’il souffrait d’une neuropathie optique bilatérale. Les caractéristiques cliniques, la survenue progressive accompagnée de très légers symptômes et la relation temporelle avec la prise d’amiodarone nous ont amenés à poser le diagnostic de neuropathie associée à l’amiodarone. Les analyses complémentaires en série ont donné un aperçu de l’évolution naturelle de cette entité. Ce patient a développé la couche de fibres nerveuses rétiniennes la plus importante dans le quadrant inférieur. Ceci peut être corrélé avec des axones de diamètre anatomiquement plus important, soutenant une publication antérieure qui suggérait que les axones des nerfs optiques de plus grand diamètre sont plus sensibles à la lipidose induite par l’amiodarone. Bien que rare, la neuropathie optique associée à l’amiodarone peut se développer et causer une perte permanente de la fonction visuelle.
