Abstract
Objectives: To determine sociodemographic features, characteristics of family and personal history, anthropometric measures, treatment protocole, phenotype and activity of inflammatory bowel disease (UBC) in participans, as well as to evaluate the impact of different lifestyle habits and environmental risk factors on Crohn's disease (CD) course and ulcerative colitis (UC) regardless of the prescribed therapy. Study design: Cross-sectional study with elements of ecological research conducted from 1 January to 30 June 2016. Participans and Methods: The study included 312 disease-affected patients, in the area of Eastern Croatia, 114 (36.60 %) patients with CD and 198 (63.40 %) patients with UC, average age 49.90 ± 15.00 years, 52.88 % of men and 47.12 % of women. The data were collected by the questionnaire and the analysis of the water safety, based on 65 samples of drinking water from the field of research. Results: Regarding demographic characteristics, there was a significant difference in age (p=0.013) and marital status (p=0.037) in relation to the type of disease. Significant difference (p=0.009) was observed in the patients’age at the time when the patients were diagnosed, and a significant difference between CD and UC compared to the presence of other diseases (p=0.037). The phenotype analysis according to the Montreal classification, the most common type UC was E2, while in patients with CD prevalent phenotype was A2L2B1. The most prescribed therapy was aminosalicylates, far more in the treatment of UC (p<00.1). There was a significant difference in meat consumption before and after the diagnosis of UBC (p<0.001). Exposure to chronic exogenous stress during the Homeland War has significantly influenced the UBC type (p<0.001) Inactive disease was reported in 66.00 % of patients with CB and at 60.00 % of patients with UC. There were significant differences in the type of therapy (p=0.016) and in the daily physical activity (p<0.001) regarding UBC course. No therapy had 24.04 % patients, 6.09 % of CD and 17.95 % of UC patients. Daily physical activity had a significant effect on CD course (p<0.001) and UC course (p=0.004) in patients without therapy. Significant difference in CD course was observed in therapy free patients who were exposed to exogenous chronic stress during the Homeland War (p=0.041). Conclusion: Observing each disease separately, the study confirmed the significant impact of lifestyle habits and environmental factors on CB and UC course in the patients, independing on the prescribed therapy. It needs to be pointed out that the patients’daily physical activity highly impacts the course of the observed UBC. This research forms the basis for the population register of UBC in Eastern Croatia.
