Le diagnostic de métastases ganglionnaires dans le cancer du sein est réalisé par la technique du ganglion sentinelle (GS) consistant à repérer et prélever le premier relais ganglionnaire axillaire potentiellement métastatique. Ce GS est repéré par injection d'un produit radioactif et d'un colorant bleu. Cependant, quelques inconvénients existent : risques de choc anaphylactique et exposition à la radioactivité. De nouveaux marqueurs ont ainsi été proposés, les Quantum Dots (QDs) qui sont des nanocristaux de semi-conducteurs fluorescents.La première étape de cette étude a été de déterminer le « coating » idéal des QDs afin d'obtenir le meilleur lymphotropisme. Trois coatings ont été testés : PEG, PEG-NH2 et PEG-COOH par injection sous-cutané (s.c.) dans la patte chez la souris. Après visualisation du GS en microscopie de fluorescence, le meilleur coating a été identifié comme étant le PEG-COOH. Quatre pourcentages de COOH : 25, 50, 75 et 100% ont ensuite été testés suivant le même modèle, la fluorescence étant suivi après extraction chimique du GS. Les résultats ont montré que, quel que soit le pourcentage de COOH, les QDs migrent dans le GS axillaire de façon identique. Une étude de biodistribution a ainsi été réalisée avec des QDs dotés d'un coating COOH (λex 655 nm). Après injection s.c. de 20 pmol chez la souris, les QDs ont été détectés dans le GS à différents temps (5, 15, 30, 60 min et 24 h) par spectrofluorimétrie fibrée, microscopie de fluorescence et extraction chimique. La quantité maximale de QDs dans le GS a été observée à 60 min et correspond à 2,4 % de la dose injectée. L'étude de biodistribution a aussi montré que les QDs étaient absents de tout organe, du plasma ainsi que des urines et fécès.L'utilisation des QDs permet donc de localiser le GS de façon sélective et rapide. Cependant, l'emploi de QDs sans métaux lourds est nécessaire pour limiter tout risque de toxicité ainsi que l'utilisation du proche infra-rouge pour une application clinique.
