Angeborene und erworbene Herzerkrankungen beim Hund : eine retrospektive Analyse

Abstract
Um die Häufigkeit verschiedener Herzerkrankungen im Einzugsgebiet des Tierspitals Zürich zu dokumentieren, wurden alle Krankengeschichten der Jahre 1995 bis 2000 retrospektiv untersucht. Bei 146 von 620 Hunden mit einer Herzerkrankung (23.5%) wurden 158 kongenitale Herzfehler gefunden, am häufigsten Subaortenstenose (SAS, 31.5 %), Pulmonalstenose (PS, 23.3 %), Ventrikelseptumdefekt (VSD,14.4 %), offener Ductus Botalli (PDA, 13.7 %) und Trikuspidalklappendysplasie (TD, 7.5 %). Rassebedingte Tendenzen waren eine SAS bei Boxern, Deutschen Schäferhunden, Neufundländern und Rottweilern, eine PS bei Boxern und Jack Russell Terriern, ein VSD bei Westies und Flat-coated Retrievern sowie eine TD bei verschiedenen Retriever Arten. Bei 474 Hunden (76.5%) wurden erworbene Herzerkrankungen gefunden, am häufigsten Klappenerkrankungen (50.4 %, v.a. Mitralklappenendokardiosen (ME), Herzmuskelerkrankungen (24.3 %, hauptsächlich dilatative Kardiomyopathie (DCM)) und Perikarderkrankungen (12.4 %, v.a. neoplasiebedingte und idiopathische Perikardergüsse). Herzneoplasien ohne Perikarderguss wurden bei 7.0 % und primäre Arrhythmien bei 8.2 % gefunden. Rassebedingte Tendenzen waren eine ME bei Dackeln, Terriern und Pudeln, eine DCM bei Dobermännern, Doggen, Boxern, Spaniels und Irischen Wolfshunden, sowie Perikarderkrankungen bei grossen Rassen. Die Kenntnis der Häufigkeit der verschiedenen Erkrankungen und rasseabhängiger Tendenzen hilft bei der Stellung einer Verdachtsdiagnose und bei der Planung einer effizienten weiterführenden Diagnostik und Therapie. To document the frequency of different cardiac diseases in dogs, all records from 1995 to 2000 were analysed. In 146 of 620 dogs with heart disease (23.5%) 158 congenital defects were found, most commonly subaortic stenosis (SAS, 31.5%), pulmonic stenosis (PS, 23.3%), ventricular septal defect (VSD, 14.4%), patent ductus arteriosus (PDA, 13.7%) and tricuspid dysplasia (TD, 7.5%). Breed-related tendencies were observed for SAS in the Boxer, German Shepherd, Newfoundland and Rottweiler, PS in the Boxer and Jack Russell Terrier, VSD in the West Highland White Terrier and Flat- coated Retriever and TD in various Retriever breeds. Acquired cardiac diseases were diagnosed in 474 dogs (76.5%). Most common were valvular diseases (50.5%, mainly mitral valve endocardiosis, ME), myocardial diseases (24.3%, mainly dilated cardiomyopathy, DCM) and pericardial diseases (12.4%, mainly neoplastic or idiopathic effusions). Cardiac neoplasias without pericardial effusion were found in 7.0% and primary arrhythmias in 7.8%. Breed- related tendencies were observed for ME in the dachshund, terrier and poodle, DCM in the Doberman, Great Dane, boxer, spaniel and Irish Wolfhound, and pericardial diseases in large breeds. Knowing the frequency of the various cardiac diseases and breed-related tendencies is helpful for making a tentative diagnosis and for planning diagnostic and therapeutic steps efficiently.
