Abstract
Tętniaki naczyń mózgowych są zaburzeniami budowy ścian tętnic mózgowych. Powstają zwykle w dużych naczyniach tętniczych podstawy mózgu. Przedstawiono przypadek pacjentki w trzecim trymestrze ciąży z krwawieniem podpajęczynówkowym z pękniętego tętniaka prawej tętnicy środkowej mózgu z towarzyszącym krwiakiem śródmózgowym. Ciążę zakończono cieciem cesarskim. Po porodzie w chora w ciągłym znieczuleniu poddana została zabiegowi embolizacji tętniaka. Rzadkim powikłaniem leczenia endowaskularnego mogą być epizody zakrzepowo-zatorowe, które wystąpiły w tym przypadku. Ze względu na brak możliwości leczenia farmakologicznego i fibrynolitycznego wykonano mechaniczne usunięcie materiału zatorowego przy użyciu trombektomu Soliter FR 3x20 uzyskując przepływ w całym zakresie unaczynienia tętnicy. Pacjentkę z dyskretnym niedowładem lewostronnym po 8 dobach od zabiegu przekazano na oddział położniczy.
