Le réseau de la santé et des services sociaux du Québec a subi une transformation majeure qui a fait passer le nombre d’établissements de 182 à 34 créant ainsi des mégastructures. Ces mégaétablissements auront à composer avec plusieurs défis éthiques liés principalement à cette restructuration du réseau et des constantes transformations : évolution rapide de la science, nouvelles technologies, de l’intelligence artificielle et des réseaux sociaux qui s’ajoute aux contraintes existantes : financement des soins et services, privatisation des soins, rapport des professionnels avec leurs clientèles, etc. Ces mégaorganisations de santé devront témoigner de la probité de leur gouvernance, des moyens et des résultats. Elles doivent innover et se tourner vers une gouvernance éthique empreinte de valeurs de transparence et d’ouverture obligeant un changement radical de la culture organisationnelle préalable à une certification éthique. Un management éthique permettrait au personnel de l’établissement de se donner l’espace réflexif et de favoriser le dialogue pour composer avec les risques éthiques et de prévenir les crises. Une certification éthique témoignerait d’une véritable appropriation des compétences éthiques du personnel de l’organisation.
