Analysis of Clinical and Endoscopic Features of Crohnʼs Disease Patients Involved in Duodenal Lesions

Abstract
Introduction: Crohn'disease (CD) involved in the duodenum is not common. Sometime duodenal lesions can be the first manifestation without other intestinal lesions, in these cases are often misdiagnosed. In order to find these CD patients with duodenal damage, the clinical and endoscopic characteristics of CD with duodenal involvement were analyzed. Methods: We retrospectively analyzed clinical data from Jiangsu People Hospital in China, between January 2015 and January 2016, and summarized he clinical and endoscopic characteristics of CD with duodenal involvement were analyzed. Results: A total of 61CD patients underwent upper gastrointestinal endoscopy, 20 patients had positive duodenal lesions (32.8%). Of these, 13 male, and 7 female, male: female 13:7. Average age of 26.3 + 14.7 years(15-64 years), one case was less than 17 years old, 13 patients aged >17 years and less than 40 years, 6 cases aged >40 years old. Montreal classification: L1:9 cases (45%), L2:1 (5% cases), L3:10 (50% cases); B1:6 cases (30%), B2:14 (70% cases); 5 cases of perianal involvement (25%). CDAI scores:150-450. One of twenty patients with duodenal lesion had stenosis as the first manifestation. 12 cases had longitudinal distribution of duodenal ulcer (60%), 6 cases (30%) patients with duodenal stenosis, 2 cases patients with duodenal bulbar mucosa like bamboo change(10%). Conclusion: Duodenal involvement is not uncommon in patients with CD, and can be the first manifestation. Patients can have various endoscopic manifestations, typical endoscopic feature of CD patients with duodenal involved is the longitudinal distribution of the ulcer, including bamboo like change.
